Whether you are visiting the hospital for the first time or simply following up post- surgery, it is advisable to keep a few things in mind for your convenience and to avoid unnecessary delays -

In case it is the patient�s first visit to the hospital, the patient is advised to arrive 15 minutes before the scheduled appointment time so that he/she can be comfortable and have enough time to fill the hospital forms. This can also be download online.

In case the patient needs to cancel or reschedule an appointment, we would be grateful if they could inform us well in advance.

Medical Records

Patient must carry a record of his/her previous illnesses and medical treatments received from previous physicians. In case they are being referred by a physician, the patient can also request him/her to send a note to the hospital directly.

If the patient has recently gone through an x-ray or scan, he/she should bring the relevant reports along.

Medications

Patient should bring a list of all medications he/she is taking currently, or has taken recently. The strength and dosage of each medication must be noted in this list.
