One of its kind surgical procedures performed in the city
Bangalore: Beams Hospitals, a name synonymous with the
revolutionary science of minimal access surgery today successfully
performed a complex laparoscopic surgery on a 46 years old woman in
Bangalore, removing her fibroid weighing 2.58 kgs – a feat achieved
for the first time by any hospital in the city. Backed by expertise
spanning over 15 years, Beams Hospitals is India’s foremost chain of
surgery centres dedicated to Minimal Access Surgery (MAS) procedures
founded in 1994 by Dr.Manju and Dr.Rakesh Sinha.
46 Year old, female a housewife was diagnosed to have fibroids in
2004, but she was reluctant to undergo treatment. (which was due to
bad experience for treatment of some ailments in past at multiple
known and established facilities in the city). She was seen in BEAMS
hospitals Bangalore OPD for swelling in the umbilicus (Large
umbilical hernia). She gave a history of heavy and prolonged
bleeding during cycles for long duration. She also gave history of
mass per abdomen and on evaluation found to have a large uterus
(about 7-8 months pregnant uterus size with more lateral extension).
She was anaemic with about 6g Hemoglobin (Normal 12-14) and was also
found to be diabetic. She was asked to have oral hematinics to
increase hemoglobin and advised diabetes control. She was planned
for Laparoscopic Total hysterectomy and repair of hernia.
However there were certain challenges in the surgery:
1. Large uterus occupying almost the entire abdomen and
abutting on the important surrounding structures which can be
damaged during the process.
2. Conversion to open as the visibility may be hampered due
to large size, bleeding and removal of such big uterus from abdomen
through such small hole.
3. Severe rheumatoid arthritis with deformities in the upper
limb and neck– difficulty in anesthesia and positioning of the
patient for surgery.
4. Diabetes mellitus and related complications
Surgery: Initially the hernia was reduced, that is the
abdomen fat stuck in hernia was removed. The Uterus had compressed
the urinary bladder and had pushed the appendix and the large
intestine up. The uterus was separated from one of the large
fibroids attached to it and later disconnected from the blood
vessels and other connections. It was removed using special
equipment called Morcellator. The weight of the uterus was 2.58 kg.
The hernia was repaired using a mesh.
Post Surgery: She recovered well and was given liquids the
next day. She started walking independently and taking regular food
from the next day. She was discharged on the third day after
surgery.
The patient (female) who underwent the surgery says, “I was
in pain and discomfort with severe bleeding that lasted almost 45
days. In the past four years, I had visited several doctors in the
city, they could not diagnose my problem correctly. Finally I
approached Beams hospitals whose team of doctors studied my case
carefully and then recommended me to go for the laparoscopy surgery.
Surgery went on for nine hours and I was very nervous. Thanks to the
competent and professional team at Beams Hospitals, I can now walk
and eat. I can now lead a normal life once again.” Patient’s husband
mentions that Previous bad experience with some medical facilities
lead us to a fear psychosis and seeing a well-coordinated effort in
Beams Team was a rare observation.
Dr.Shanthala, Laproscopic Gynaecologist, Beams Hospitals says,
“Generally hysterectomy is performed for a wide range of benign and
malignant conditions, such as fibroids, menorrhagia and pelvic pain,
and gynaecological malignancies. One in four women has a chance of
undergoing hysterectomy in her lifetime. Conventionally hysterectomy
is done by most gynecologist’s through the open approach. Total
Laparoscopic hysterectomy is superior to the standard approach but
is less prevalent due to scarcity of trained laparoscopic
gynaecology surgeons. It is added advantage of smaller incision,
faster recovery ad return to normal life”.
Commenting on such surgeries, Dr. Anand, General Surgeon,
Laparoscopy, (who helped Dr .Shanthala in hysterectomy besides
Hernia repair for this patient) Beams Hospitals states that every
year in the United States about two-thirds of hysterectomies are
performed via the traditional abdominal route. Furthermore, well
over 90% of hysterectomy specimens weigh less than 460 grams.
Laparoscopic surgeries have become order of the day for surgical
procedures like hernia, appendix, gall bladder removal and the trend
of Lap Hysterectomy is fast picking up in US and in India as well.
