Case Study - Dr. Shanthala Thuppanna
40 YEAR OLD OBESE WOMAN WITH A LARGE UTERINE FIBROID AND LARGE PARAUMBILICAL HERNIA
The Case
•   40 year old lady from Dubai
•   H/O severe bleeding during periods
•   Large swelling in the navel
•   Advised, earlier, open hysterectomy with repair of hernia as she was obese (BMI 37) with large uterus
•   History of previous cesarean section and also large hernia.

She consulted the Gynecology and Laparoscopic Surgeon at Beams hospital, Bangalore and a combined laparoscopic hysterectomy with umbilical hernia repair was planned.

The Challenges
1.  Obese patient, large uterus, large hernia.
2.  Doing a combined procedure like this would result in problems like prolonged duration of surgery      leading to deep vein thrombosis, pulmonary embolism, pneumonia.
3.  Increased pain and delayed recovery if done by open method due to large incision.
4.  Chances of mesh infection as the wound is kept open for a prolonged time during removal of uterus      because of which a separate hernia repair has to be planned.

The Procedure
Total laparoscopic hysterectomy was done and the uterus (weight around 1 kilogram) was removed through a small incision using a morcellator (cuts uterus into pieces and suck it out) later the umbilical hernia was repaired using a large dual layered mesh .The combined procedure was completed in 3 hours.

Recovery
Postoperatively, the patient was managed under the care of the operating surgeons and the intensivists. Patient was mobilized on the same day. Patient started oral liquids on the second day and solid food on the next day. Patient was discharged on the third day.

Follow-up
Patient came for follow up after a week and had started her regular daily chores at home. She has now gone back to Dubai and is doing well.

32 YEAR OLD WITH LARGE SWELLING IN ABDOMEN AND HEAVY FLOW DURING PERIODS
The Case
•   The patient presented with a large swelling in her abdomen starting from her pelvis up to above her      navel
•   She had heavy flow during her periods and was anemic
•   She had been advised hysterectomy as the fibroids were huge

She visited Beams Hospital, Bangalore and was seen by the Laparoscopic Gynecology Surgeon and was advised Laparoscopic Myomectomy so that her uterus could be preserved as she was only 32 years old.

The Challenge
•   Large fibroid occupying almost the entire abdomen

The Procedure
Laparoscopic myomectomy was done and two large fibroids were removed and the uterus was sutured. The fibroids weighing around 1.3 kg were removed using a morcellator.

Recovery
Patient mobilized on the day of surgery. She was discharged on the 3rdpost operative day.

47 YEAR OLD WITH MULTIPLE FIBROIDS
The Case
•   Patient had a history of excessive bleeding during periods for 4 months
•   Huge mass in the abdomen
•   She was diagnosed to have large multiple fibroids with the uterus occupying her entire abdomen
•   She was advised outside for an open hysterectomy in view of the large size of the uterus

The Procedure
Total laparoscopic hysterectomy was done and 1.75 kg uterus was removed through a small hole using a morcellator.

Recovery
Patient recovered uneventfully and was discharged on the 3rd post operative day.
